  24. two physicians who have personally examined me, one of whom
  25. shall be my attending physician, and the physicians have
  26. determined that my death will occur whether or not life-
  27. sustaining procedures are utilized and where the application
  28. of life-sustaining procedures would serve only to artificially
  29. prolong the dying process, I direct that such procedures be
  30. withheld or withdrawn, and that I be permitted to die
  31. naturally with only the administration of medication, sustenance,
  32. or the performance of any medical procedure deemed necessary to
  33. provide me with comfort care.

  37. regarding the use of such life-sustaining procedures, it is
  38. my intention that this declaration shall be honored by my
  39. family and physicians as the final expression of my legal right
